 Why GPs refer to us?


Tele- health / Ka Ora. 

Routine assessment and symptom monitoring for patients assessed via virtual providers that require in person assessment, treatment, or intervention. 


Post-Acute / Transitional Care. 

Rapid follow-up after hospital discharge or acute illness to ensure safe recovery, preventing complications.


Safety Net Follow up. 

Timely escalation of care for high-risk patients recently assessed in clinic. Early monitoring reduces risk and ensures escalation if needed. Patients are monitored between appointments, ECPs handle follow-ups, freeing GP capacity.
 

Medication & Symptom Monitoring. 

Support for patients on high-risk medications or complex treatment regimens, including adherence, review and patient education.      



  • Structured reporting: clinical assessments are documented and sent back to your practice.
     
  • Flexible, prompt service: home visits within 24–48 hours, routine follow-ups within 5–7 days.
     
  • Integrated with SR Referrals: refer electronically directly through your GP practice system.


How to refer?

Referring a patient is simple: Ask your GP practice to request Paramedic Services from their SR E-Referrals listing.

1). Select the patient in your PMS and send the SR referral electronically.

2). Our ECP team will schedule a home or clinic visit and send feedback to your practice.
